Now that you’re ready to use your Ambetter from Arizona Complete Health benefits, take a few first steps to get started.
Use your member account to download your ID card, activate your My Health Pays® Rewards account and more. Start using your benefits and preventive care services and schedule your first exam to stay healthy.
Download Your Member ID Card
The easiest way to access your ID card is in your Ambetter Health online member account. There, you can also request a paper copy.
Activate your My Health Pays® account
Start earning rewards points! Through My Health Pays® you can earn rewards for completing healthy activities, such as eating right, moving more and living well. The more activities you complete, the more rewards you can earn to use towards healthcare-related costs. Learn more about My Health Pays®
Take Your Member Survey
Complete your Ambetter Health wellbeing survey in your member account. Then, we can identify additional programs that may help you manage your health. Once you complete the survey, you can earn rewards points!

Find your primary care provider (PCP), or main doctor, using our Find a Doctor tool. Schedule your first annual exam – it‘s free and it’s covered. After you complete your first annual exam with your PCP, you can earn even more My Health Pays® rewards.

Healthcare-related costs will vary by member and the plan in which you are enrolled. Funds expire immediately upon termination of insurance coverage. My Health Pays® rewards cannot be used for pharmacy copays. My Health Pays rewards cannot be used to pay premiums. Restrictions apply. Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all members. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
